Where to find your medication receipt
Your official prescription receipt is included inside your medication package. This receipt contains all the information you need to submit an insurance claim, including:
- Drug Identification Number (DIN)
- Prescription/Rx number
- Dispensing fee breakdown
- Drug cost breakdown

Submitting an insurance claim
Our clinic is unable to bill your insurance provider directly. However, many of our patients successfully get reimbursed for both the consultation and medication costs through their insurance. Here's what you can submit:
- Medication cost: Use the official prescription receipt enclosed in your package. It includes all the details your insurance provider will need (DIN, Rx #, drug cost, and dispensing fee).
- Consultation fee: If your benefit plan includes a flexible health spending account, the $39 consultation fee is typically reimbursable. Submit a request to receive a consultation invoice.
Pharmacy information for your claim form
If your insurance provider asks for pharmacy details, use the information for the pharmacy that dispensed your medication as indicated on your prescription receipt.
